The Arbiter of Ash is no longer accessible after only 2 attempts

Hello,
After gathering the 3 crisis fragments, I was only able to do 2 attempts instead of the 6 planned for this boss. After my 2nd death, I resurrected at my lair, my 3 fragments were consumed and the door to the monilith is now permanently closed.
What to do??
