Citadel Failed bug

I've defeated all the rares and bosses in the Stone Citadel, yet it’s marked as failed, and all the connected maps are inaccessible.

Bump.
Experienced the same bug. Killed the boss, dropped 2 fragments as well, got the map completed notification in the map (killed all rares before boss as well.) yet it shows map failed and cant go to adjacent maps. |

Having the same issue, killed boss and got the fragment but showing up as failed.
This was a "Stone Citadel" and the first citadel I've completed for my character. |
