Weapon and support gems disappear when using equipment button with full inventory

Apologies if it's been posted already. Twice I have accidentally pressed x on controller to equip a weapon from my inventory and had the equipped weapon disappear. It happened when I had a completely full inventory and had support gems in the weapon skill. The gems were also lost, the accidentally equipped item was put in slot successfully but the existing item and gems disappear. I checked my stash, the ground, and even doubled back to make sure it wasn't dropped. My inventory had zero spaces available and it was the same weapon type I was sorting my equipment in a map after clearing. I play on PC with a controller and equipped the kept item by pressing x
