Quest Item Not Dropping in Venom Crypt

I went into the Venom Crypt to do the quest but realized I hadn’t received the quest yet. I cleared the cave and noticed the quest item didn’t drop. After going back to town, I got the quest from the quest giver and went back to redo the objective, but the item still didn’t drop.

Now, my quest log shows the quest as completed, but it also says it’s unfinished. Now i can't get the 10% chaos resistance from the quest anymore.
