No loot dropping

250 hrs in, on my 4th character, started to notice that bosses weren't dropping loot but I believed it to just simply be RNG. Completed Trials of Sekhemas, opened chests with remaining keys, no drops. There was quite a few times the chests didn't give me rewards before on previous characters and majority of the time there was drops so once again I believed it to be RNG once more. It wasn't till after I completed the Temple of Chaos that I was 100% sure something was off. Claiming rewards all I got was a single soul core and no rare drops. Is something wrong with my character or game file?

Is it possible that you have a loot filter applied and it's only showing the most valuable loot? If you hold down alt or z (defaults), does that help at all?
