Ignagduk, the bog witch does not give spirit (Cruel)

Act 3 Boss (Cruel), Ignagduk is not dropping anything and the map icon is still not dimmed. I remember getting the +30 Spirit in Act 3 (Normal), but does the Cruel icon dim for anyone else for Act 3 Cruel? I have killed her several times to see if it goes away and it's not for me.

I killed her and got the item that gives +40 spirit this time.

However, the enemy that bugged for me in this very same way is ROOTDREDGE, that necromancer tree from Sandswept Marsh.

This also happens on my quest from act2 Ascent to Power - Search Traitors Passage

I encountered same thing as a monk.
Have tried many times killing her but end up with no drop.
Any idea of how can I avoid this?
