Path of Exile 2 Sacrificial Heart Issue Act 3

I have done acts and through act 3 i didnt knew what i do with heart .. i throw it then at act 6 i did the quest and i had my 2 passives. i return to do it on act 3. heart is not dropping for me it seem its bugged

i did Utzal 4+ times and i did Aggorat on act 3 mutliple times i cant not find it

now im stuck without my 2 passives.
