Bug: can't unlock 4th ascendancy

My game crashed after completing my 4th trial. I tried to capture the video upon killing the final boss on Xbox and that caused my game to crash.

When I logged back in I was at the waypoint without receiving my points. I have since completed it two more times and I still do not get the ascendancy point rewards.

I was using item level 75 inscribed ultimatums that state "will grant ascendancy skill points" and I have already completed the other three trials so I am sure it has something to do with my game crashing after I completed the trial the first time but before I received the skill points.

Is there anything I can do to fix this?

My game didn't even crash, it just doesn't show me the shrine lit up that I can click on. Beat a 10 level in chaos temple and I don't get to ascend. WTF

You need to beat the Trial master in the secret side room at the end of the 10 total trial version, which requires the three pieces to unlock the room. So basically run the trial a few times, to get a chance at killing him for the ascendancy or run the Sekhemas trial with 4 floors to get the 4th.
