Game Disconnects When Swapping In and Out of Demon Form

Started in Cruel difficulty when fighting the boss in Act1 Mud Burrow.
Changed to Demon Form and game disconnected. Decided to call it for the day and played something else.
Came back to the game today and found that whenever I would swap forms it would disconnect me immediately and start me in the form I changed to when I reconnected.
Game seemed to be saving a snapshot where I left off each fight so I could finish the battle.
Confirmed it continues to happen after finishing the boss so my game is fairly broken now.
I figure you can look up the character associated with my account and get a better idea of my gems/build but if you need me to post any of it just let me know.
