Dealing with Freeze (the Ailment)

How is everyone dealing with Freeze in maps? It's probably the biggest culprit when it comes to dying in high-end maps, at least for me when playing melee. This is especially true because a lot of maps on the Atlas like to spawn Vaal Shades who cast Glacial Cascade (along with a plethora of other sources of heavy chill and freeze from rare affixes, etc.). You can dodge as many freeze sources as you can, but you'll eventually get caught out and frozen - which often leads to death. Thawing charms only help so much in maps with high density, and there aren't many other sources of Freeze cleanse or resist. Is the answer to anoint Warm the Heart onto your amulet (reduced freeze duration and increased freeze threshold)? How are other melee players handling getting frozen?
