Using demon form and turning back to human form disconnects immediately

Recently, around the time i reached the end of the 3rd act, using demon form started disconnecting me with the messsage "Unexpected disconnection occured", after logging back i continue from the exact place i was disconnected - be it boss fight or just some stuff, trying turning back to human disconnects again and i have to log in again. Has anyone encountered a similar problem?

Having the same issue for the 2nd day. Yesterday it somehow got fixed but today it began again and i disconnect all the time. Removing one gear when switching sometimes works but not consistent. Just unplayable.
