Ascent to power quest not clearing ACT2 Cruel.

I've done the first 2 ascenancies, on act 2 cruel, was given the quest again, cleared traitors passage and killed balbala again and the quest wont clear.

Also on a side note how do you get the 3rd and 4th ascendancies? / what level are they aimed for?

I have the same problem with one of my characters, in the second act cruel shows me that I haven't done the ascent to power quest yet, I thought I hadn't killed her, to be sure I went to a quick meeting with her. Unfortunately, the quest didn't pass and I still can't get it :/ As for your question about 3rd and 4th ascension, you have to do them after getting the token in locations 60+ and 70+, I think.
