Permantly missed skill points from quest

My warrior had the sacrificial heart in the inventory before going through the portal to cruel difficulty. Upon going through the portal, the quest item somehow disappeared. I was not able to get it again with multiple runs in Utzaal and locked out of the skill points from the quest. Please help.
