How do you get more charm slots?

Haven't seen a belt yet with more then 1.

suffix mod on belts, requiring level 55 for the first tier (+1 charm slot) and 82 for the second (+2 charm slots)

incorrect, i found a +2 charm slot belt that requires level 51 (item level 77)

3 charm belts ony cost 1 exalt.
