Sacrificial Heart disappears from inventory after Act 3 - Unable to obtain a new one

I obtained the Sacrificial Heart quest item in Act 3 and completed the Act without turning the quest item in. When I began Act 1 Cruel, the Sacrificial Heart was missing from my inventory.
I have returned to the Utzaal zone in Act 3 and cleared nearly the entire map, but a new Sacrificial Heart will not drop from enemies. As it stands, I have no way of retrieving a new Sacrificial Heart and have missed out on 2 passive skill points that I should have. I feel like this is a critical issue that needs to be addressed. You've always been able to hold off on quests in Path of Exile and complete them at any time, and certainly have been able to reobtain quests items that have left your inventory.
