Trial of Chaos Cruel Bug

Okay so Today i got the quest for the Trial of Chaos on cruel difficulty.
I went there and noticed that the quest item was gone. i really dont know when and where. So i just tried to do some other runs.

I cleared about 5 Trials on different level caps and the quest wont complete. So im missing 4 Ascendancy passive Points by now.

Somebody had this happening to them and know what to do ??
