"The Mysterious Shade" quest bug - gamebreaking

Hi, got a bug that is preventing game progression on 2nd character.

I've finished this quest without any problems on my main character.
Then my wife joined me via couch co-op. We've then progressed together to the point where her 2nd character have pretty much same progress as my 1st character.

However, quest "The Mysterious Shade" is now stuck at phase "Bring Count Lachlann's Ring to Una" for that 2nd character. Una is not standing in the tent in Clearfell Encampment, therefore no quest option. When traveling to TheGrelwood where The Hooded One is hanging on that tree, there is an option to summon Una - she spawns, says "What happened here" and disappears immediately. Doing this in new instance have same result.

I think what happened here is that somehow Lachlann has been slain before the quest was properly activated at this place in The Grelwood - I don't recall being there with 2nd character when we played together.

Any idea how to fix this?

Same exact problem for me too. I also tried dropping the ring back in the cemetery where it's picked up to try and somehow reset the issue. This only seems to break it more... as I return to collect it there are now two instances of the ring on the ground, then 3, then 4 and so on. I also tried dropping it and then logging out and back in but that does not fix it either. This character is stuck. Having some way to reset/restart a quest would be huge for this type of issue. Thanks.

Do we need to restart from scratch? I mean new character

12/28 this is still happening

(2-1-25) still having this issue. my wife is unable to continue, with or without me.( sometimes with quest bugs we’re able to quit the game and have her load in without me and it fixes it) but i’ve tried everything and no dice.

Just to be sure, is her character unable to complete the previous steps of the quest?

Collecting the runes from the Red Vale, giving them to Renly to get the spikes in return, and using those spikes to release the Hooded Figure from the Grelwood?
