Beira of the Rotten Pack quest reqard bug

Killed the boss, went back later realizing I didn't get the 10% cold resist reward. Killed her again, no wolf head. Checked in town to see if npc had the head. Bug?

Same, it disappeared in act 2.

Id like to recant my some of my former reply, I had the wolfs head and used it. THE MAP IS WRONG though. It shows as I have not completed it hence the confusion.
