Azarian, the Forsaken Son Bug After death

I died while fighting the boss but had a chaos dot on him. So the boss died while my death screen was showing up. After respawn at checkpoint, the boss was lying on the ground dead, but the dialogue was going as if it were alive. The loot disappeared but i was able to get to the goddess.

However the arena was locked as if the boss fight was ongoing making it impossible to go back

Similar, but different process.
Same end bug with the arena door, but do you also not have the boss registered on the map as defeated?

Azarian, the forsaken son" not registered on map as defeated.

During intro speech, i could curse him then damage him, this led to cutting off the voice lines.

afterwards, dying to the many many statues on the other side after opening the sarcophagus,
the intro speech for "Azarian, The forsaken son" and the water goddess continued from where it was cut off before the fight.

Walking back to see if the boss revived, voice line "my king nears!" plays. The boss is still dead, but the quest to talk to the goddess and burn her has refreshed.
Door out of the Arena is locked.
Still not registered as defeated on the map..

Playing on sorceress if that matters.

Sorry if to much information, but figured it might help in pinpointing why this happens.
