I have killed Beira of the Rotten Pack multiple times but am not getting credit for the kill so can't get the upgrade.

Me too !! I got the same bug. I killed her 3 times now... still not getting any upgrade :(

Actually nvm, the reward showed in my inventory. I right-click to use it and the skull icon greyed out.
