Hard to tell. I guess its a combination of some crazy map mods with a bunch of crazy rare mobs.
Its not always one shot more like one sudden burst of damage by multiple mobs. The best way would be record and analyze game. |

No "suppress Spell damage" some mobs with spells can kill me on one shot even with 7500 life on T10 maps with 0% suppress. No crazy map modifiers.

you need 100% in every category.
There is not much complexity to it. the damage is scaled so high that if you dont you are dead. So go get 100% in everything. It should be the standard answer in build help What shoud I do ? 100% | |

It appear you have very little recovery, the only recovery I can immediately identify is 47 life gain on block and vitality buffed by a bit of regen on the tree.
Have you tried adding leech and life gain on hit to the build? | |
