Split into three bug

I used the split item into three copies with two mods craft on this wand:

I still have the wand and it is unsplit. I have lost my Black Morrigan beast and received no split items. Even though Einhar said "You are truly a survivor!" and the animation played for winning, nothing happened. Neither me nor my friend died.
After looking into it, I thought it was because the wand doesn't have 6 affixes but if that was why then the recipe would've stopped me from doing it, right?
