Windowed Fullscreen bugged on startup

When I start Path of Exile (regular, not Steam) the game starts fullscreen but then immediately goes to 800x600 resolution and sits in the upper left corner of the screen. Otherwise displays fine. I can go into the Options and toggle it to Windowed. Then it stays at 800x600 but gets the Windows Frame. After saving that and switching it back to Windowed Fullscreen it switches to the proper 4k resolution and stays there.

I can't get it to save this and start properly? I recently added a 4k LG C2 OLED TV to my PC for gaming. Never played PoE on this TV before, so not sure if its a new thing or old.

When I switch it to Fullscreen it seems to be uncertain which Monitor to start on. I have 3 plus the TV, but I cannot get it to properly start on the TV every time either. I prefer Windowed Fullscreen anyway.

It is slightly annoying having the to "reset" this every I start the game, so help would be appreciated.
