Blood and Sand has no visual effect (mtx broken too)

It's been ages since I used this skill but now it seems to not be working properly. Video below shows the gem working but no cosmetic either default or purchased mtx working properly. Also showed at end of video that other mtx seem to be working fine.

Playing on a PS5. Please let me know if there is any other info needed.
