Nameless seer didn't show up after full clearing map

Did a t17 map, fully cleared the entire map where there was 0 monsters remaining. One of my mobs has the modifier "Final Pack spawns the nameless seer on death." Following the entire map being cleared the nameless seer did not show up. I have video recording as proof as well.

Really lame bug, first time seeing him after 100's of maps and I dont even get to open the shop :/

Confirmed, this also happened to me on a t8 map.

Same thing happend to me on t16 Stagnation just now. Roamed the map after having 0 monster remaining, map overlay open, did 6 laps atleast - checking every single corner etc. nothing...

Same right now. Cemetery T16, 0 monster remains

I've had this happen in The Belly of the Beast lv2, maybe because I think the blood monster I put him under never spawned in the area, or at least I didn't see any (which shouldn't be allowed tbh)
