Perfect Forest Warrior Doesn't Reapply Onslaught After Death

The Perfect Forest Warrior supplies an Onslaught buff to you and nearby allies when raised as a spectre. If you die in a map for whatever reason and come back into the map, the Perfect Forest Warrior doesn't reapply the Onslaught buff to you.

This bugs for that entire map. Leaving the map and coming back in has no effect. Logging out and logging back in has no effect. Returning to hideout, removing the Raise Spectre Gem and resummoning the Perfect Forest Warrior then reentering the map does not fix it. Resummoning from a different Perfect Forest Warrior corpse does not fix it.

Basically if you die at all in a map, you will never get Onslaught back in that map from the Perfect Forest Warrior.
