if a rare item has 1 empty prefix and 1 empty suffix, can i add 1 prefix and 1 suffix at the crafting table? eg. life (prefix) resistance(suffix)

Only if the item has "Can have 3 crafted modifiers" craft on it.
Normally you can only craft 1 modifier. |

As above says, you will need to use the "can have up to 3 crafted" mods affix to have more than one crafted mod the item. It is in itself a suffix
