Ritual Items don't disappear the way they should

"This is the last time this item will be available from an area, unless it is deferred again"

I've been monitoring Rituals for a long time and there's definitely something broken here. I didn't defer either of these red items last map, and here they are again.

In fact I had an item (Prime Chaotic Resonator) that continually appeared 3 maps in a row, all the while being red and supposed to disappear. Never deferred it once. I know it was the exact same resonator continually reappearing, because I was juggling 3x of them (each one has a different base price and fixed price increase based on that base price).

I have all atlas tree Ritual nodes, and I always reroll 3 times on the final Ritual.
