How exactly do I get the 5-slot map device?

The wiki says that I have to get 4 timeless emblems into the map device. Ok. Did that. Died with seconds left on the clock and tried to respawn and rush back in. Got nothing, not even drops.

Some posts I've seen says just starting the 4-way is enough. It's not.

Others say I have to be in the realm of timeless conflict when the counter runs down, even if I'm dead I still get it.

Another poster said I have to survive and kill at least one boss.

I've re-speced my atlas passives for full monolith hunting, and I'm not working from zero thankfully, but I DO NOT want to grind up 400 fragments for a third time.

So. Please. What EXACTLY do I have to do to get a 5-slot map?

If you walk into a boss room after the boss has been killed, you don't get the clear. Similarly for legion, you need to be in the map / zone when the timer hits zero.

If you die during the encounter, then stay dead. Let the timer hit zero, then respawn.

4-way is enough.

I wouldn't leave right when it hits zero wait for items to drop then respawn.
