5 lvl awakened elemental damage with attacks doesn't prevent you from dying to ele reflect

I am 100% sure it's the reason of the rips. I am not sure what is causing that or how to replicate it, but I think it's something related with freshly spawned mobs you kill super quickly as they spawn (breach, legion, scourge etc.). If it indeed doesn't work at all times, it's super dangerous for any hardcore players trusting the gem.

Every time when I run reflect map with awakened elemental damage with attacks I die 1-2 times. This deaths not because mobs hit me or sometimes mobs are in offscreen so I think "cannot be reflected" mod on this gem does not work as it should

No, unless you count herald of ice, but iirc it cannot be reflected.
