Maddening Presence - Impresence

So i didn't pay too much attention to this buff till today.

Im not obtaining Maddening Presence after killing rare enemies, only after killing unique enemies, and there's also problem cuz i don't get buff always, just sometimes, its hard to say if it's graphic bug or item is bugged.
