Stuck at Vault Doors - Cannot Proceed to Maps

So this happened. I was at the vault doors and I could turn either of the keys and the door would not open. I reset the game, restarted the instance; everything I could. I was stuck and could not progress to get my map device. Order of turning the keys had no impact. The door simply would not open.
The way I got around it was to have another player open it, but this is a major problem that blocks players from mapping. Screenshot included.  All that and a bag of chips! Last bumped on Dec 15, 2019, 8:43:21 PM

Talking with more people, apparently you must hit both keys within 3 seconds, but I am 99% sure I tried this because I thought it might be a combination puzzle (left then right, or right then left then right). Since it's done I can't test it, but I'm very certain there was a bug here.
Separate from the bug issue, this "design" is terrible; the player is never told or notified they must activate both keys within a timeframe. PoE is NOT a puzzle game. This design does not belong in the game. All that and a bag of chips!

I had to try to get these open like 5 times, yes, the first still has to be working when you hit the second. But yeah, I think there is a bit of a bug or it needs a longer time as I'm sure that I did it right at least 4 of the 5 times nad only the last worked
