Graphics Glitch or lines flickering all over the screen

Hi

i have been facing this issue for a while i cant play the game due to this
5 to 15 mins after starting to play if i did not crash the game just start to give me like some sort of graphics glitch or flickering textures and lines all randomly all over the screen , i checked my CPU and GPU temps they are fine , and this happens only in POE , tried it in several other games like WOW and division 2 , all goes well.
also i would like to add that this happens only in game , on windows desktop everything is normal

and it always either do that or crash and give me this error

exception:
create swap chain for hwnd: the gpu device instance has been suspended use getdevice revmoed reason to determine the appropriate action
(getdeviceremovedreason: the gput will not respond to more commands, most likely because of an invalid command passed by the calling application)

so its either this error or the messed up lines and flickers or both.
and again this only happens in POE

all my drives and windows update is to the latest

specs:

CPU: AMD 8350 8 cores
GPU: gigabyte nvidia 1080 gtx aorus
rams: 12gb crossair
PSU: FSP Hyper 700

i wanted to provide my dxdiag which shows no issues as well but i dont know how to attach a file here

please if any can help would appreciate it , the game is unplayable because of that it hits me just 5 to 15 mins in game.
