Did not complete quest after killing Arakaali (2,029,943,571)

Just killed Arakaali, Sin swooped down, Yeena appeared, her corpse is in the middle of the room. I have the waypoint for Act 8 Ramparts, but I can't zone into it because I do not have the required quest state. I didn't zone out when she died or anything, I was there the whole time. I was not in a party, and have not been in one for a few hours.

Killed her again and got it this time. Also like to add that she didn't drop any loot the first time. Looks like the game spawned all the stuff that happens when she dies without her death actually occurring/triggering.
