Scion Raider Ascendancy "Onslaught while on full frenzy charges" not working

You have Onslaught (your attack, cast, and movement speed are increased), but the Onslaught is not granted by a buff, therefore there is no buff icon.

I wouldn't trust your DPS numbers as they will change as a result of your frenzy charges. Look at your movement speed, that should be the clearest indicator.

Yup, working as intended! You have Onslaught in the first two pictures, but not in the third.

A buff icon is only shown for buffs that are affecting your character. In Path of Exile, a Buff is something more than just a "positive effect on your character". Buffs carry with them special mechanical interactions; mainly being boosted by "increased effect of Buffs" modifiers.

Onslaught is not a buff; it is just a collection of modifiers. (20% increased movement, cast, and attack speed.)

The Onslaught support gem grants you a Buff, whose only purpose is to give you the Onslaught effect. You see the icon of this buff, not an icon for Onslaught.

Similarly for a Silver flask: you will see an icon for the flask effect, which is different from the Onslaught support icon. This flask effect grants you Onslaught.

With the Scion/Raider ascendancy, you are simply granted the Onslaught status while you are at maximum frenzy charges. There is no buff or another effect that has a visual indicator that is granting you Onslaught, therefore you will not see any other icon besides just the frenzy charges.

Eh, could be worse. It could be the "why am I dealing no damage after taking a passive that says 'deal no damage'?" that is about as frequent.
