Create Unique Map

I just did the create a unique map option with the crafting feature, nearly died, and did not get any unique map in return. Is this going to be fixed and am I going to be reimbursed?

" One thing is for sure - you wont get reimbursed. A well known fact that if you lose your character or any item due to a bug or glitch GGG can't do anything about it. |

Just had the same thing happen to me. Won the fight, animation happened for the altar, but no reward.
