Killed, not likely, no sleep for 5 days is not healthy, but it will not kill you. Not drinking water during the race may. You would be in a happy place around day 3. By day 5, you will be temporarily insane. I would love to see any streamers in the top of hardcore leader boards rage from having their character die on day 4.
Could you get hurt though, yes. Thankfully they have the level 70 raffle thing. Most will give up and sleep. A few folks will grind on, but you could eat and drink while grinding, if you do not it is not the fault of GGG. It is just evolution doing it's thing.

Chris, it concerns me that GGG added 6 hours to the end of a 1 month race with 9 hours remaining in the race. Everyone that I talked to in the top 20 was thrown for a loop by this addition of time randomly and is not really sure where this came from, nor were any of us prepared.
The timing of this addition of time is really what most of us find weird/bad. Many people planned to grind based on how much time was left and this threw a lot of schedules off. Ultimately, this could change the top 20 ladder in a way that people were not prepared for before this announcement, especially for people that didn't see the announcement because of sleep/work/etc.
I think something like this is perfectly fine if done far enough in advance though - a few days maybe instead of 9 hours before the end. Please try and wait until a time like that next time so those of us playing for top spots can plan a little better - it would be really appreciated. Thank you :)
I'm Sorry if this is a little confusing - I'm quite tired and just woke up from a nap.
nobody is even on and mapping right now, and they won't be for another 4 hours or so. i don't understand the time addition either, but it doesn't really make a difference.
