Friday Sept 16 NZT Site Outage - Now with Incident Report

We have completed our initial investigation and I have prepared an incident report about this issue here:
http://pathofexile.com/forum/view-thread/7869/page/3/#p143772 3.5 hours ago, the Path of Exile site and Beta realm became unable to retrieve information from one of our database servers. This led to people being unable to log in to their accounts on either the site or the game. This has now been fixed and will be investigated fully.
