Golden Hand to complete the quest "Through sacred ground" didn't appear

Hello.

Act 1 nemesis. As I clicked the altar for the golden hand a spider attacked me, and the golden hand never appeared on the ground, but the altar was made non-interactable. I ran the map again hoping it would show up again, but the altar was just clickable with no quest reward. The screenshot is from the second run. Would really appreciate some help to get my refund points. If this is me messing up some quest order my apologies in advance. Thanks for the help.
