Constant lag "ticks"

Have played the game smoothly in the past, but not for quite some time. Recently booted it back up and made a fresh character, and the game constantly has little lag hitches around every second. Not seeing any other internet congestion on my end of things, and it's definitely not a hardware issue. Are the servers being overloaded?

Since what you're experiencing is a bit vague..

In game:
Options > Input > Toggle Performance Metrics
What part of the graph is showing an issue?

If you're experiencing a ping issue:
https://www.pathofexile.com/forum/view-thread/1377789

If your ping in game is solid and you're still experiencing "hitching or spikes", then look at one or a few of these things:

Swap off DX11 to DX12 or Vulkan

NVIDIA drivers newer than 561.09 have known issues
This is easily searchable by the Reddit NVIDIA Megathread (insert driver version)
using an app called DDU (display driver uninstaller) while in safe mode is the best way to roll back to an older NVIDIA version

AMD GPU issues can come from Adrenaline or its overlays

Steam Overlay can be problematic

Windows 11 version 24H2 = known issues with stuttering and crashing.. try to run 23H2

Overlays from other apps can cause issues with games causing stuttering and crashing.

Try deleting the Path of Exile Documents folder to let the game rebuild
Back up your production_Config.ini first in case this doesn't help
Documents > My Games > Path of Exile

Worst case a complete wipe & reinstall might be needed

I'm also having issues - never before had them but now the game is unplayable due to CPU usage.

I've seen your posts. No information included in them except for "is this broken" "same issue"

Did you check anything in my post above? Does anything in the post above match what you're running?

I don't have high usage so it's not everyone.. The issue you and the others in these threads are the minority of players and it's possibly caused by something I've listed above.

Just list your PC specs or link a DxDiag to https://pastebin.com/
I've tried all the suggestions in both threads. I have played multiple leagues of POE on this computer without any hitches, which is what would suggest to me that this is caused by the game itself.

Here are my specs:

Operating System: Windows 10 Home 64-bit (10.0, Build 19045)
Processor: Intel(R) Core(TM) i7-8700K CPU @ 3.70GHz (12 CPUs), ~3.7GHz
Memory: 16384MB RAM
DirectX Version: DirectX 12
DirectX Database Version: 1.0.8

Card name: NVIDIA GeForce RTX 2080
Display Memory: 16159 MB
Current Mode: 3840 x 2160 (32 bit) (144Hz)
HDR Support: Supported
Output Type: Displayport External

Looking at the graphs, it looks like it's a ping issue, the ping line is peaking cyclically about in time with the lag I'm experiencing; even just standing still it'll just jump from ~50ms to ~300ms on a cycle. I'll take a look at the other threads you linked to, thanks!

When you're having the issue is when you want to reference the GGG post on WinMTR.
Just follow the instructions they list and link the pastebin link back.

There can be other causes also, snip/clip that performance graph.
(Win Key + L Shift + S)
then paste it somewhere like imgur.com or paint on your PC.

Here's the WinMTR report: https://pastebin.com/raw/pmrnYbwX

I don't know enough about network behavior to diagnose this myself from this information I'm afraid. I'm running the game through Steam, if that provides any useful information.
