Rare items at vendor

Today I bought a rare (yellow) item, I think it was a helmet, from an NPC, when looking thru all their wires.

Was this because someone else sold it to the NPC (I think maybe A3 or A2), or is there a random chance for them to spawn yellow items along side their usual inventory of blue and gray items?
