Delirium timer suspended and reward never given, no Unending Nightmare

I was rushing to boss in Basilica in Delirium fog

after killing boss, AND clearing the map, the reward didn't drop (timer just suspend indefinitely). As in picture, I did not have Unending nightmare, nor do I have Imagined Pursuits

Was it an Elder Guardian map or just a normal Basilica map?
