Enter sanctum in Standard League?

I only play standard league since I don't have enough time to play leagues anymore. Wanted to see what sanctum is about, but how can I enter the sanctum? I've got a quest that says 'Place the forbidden Tome in the Relic Altar and then place a relic', but I have no Tome, no Altar and no relic. There's also zero indication on where I should get those things. This has got the be the most badly described 'quest' in any RPG.

In Act 7 in Fellshrine Ruines is an Entrance where you can find the relic altar but no idea where you can get the tome, i search also.

1. Go to Oriath Docks in Act 10. Speak with Divinia.
2. Go to Ossuary. Find Forbidden Archive in Ossuary. Pick up the tome inside. 3. Go back to Divinia in Oriath Docks in Act 10. Talk to her and there is option to travel to the Sanctum. 4. Travel to Sanctum and place the tome in the altar. |

Does anybody know how it works nowadays?
I started the new league (settlers) with a character, made it to the forbidden archives, found the book and could "unlock" sanctum. However, I started a new character, went to forbidden archives but the chest does not drop a (new) book. Therefore I cant unlock the sanctum with my second character. Any hints? What am I overlooking? Thanks in advance. (c) Kroax
