Chaos recipe fail and I dunno why

Noob that has been grinding his way up solo has a noob question for y'all: I can't seem to get the chaos recipe to work. Here are the items I'm trying to convert...




Item levels range from 64 - 72, so my understanding is I should get a chaos orb out of it. I'm sure there is something glaringly obvious that I am missing, but, alas, I'm missing it!

You guys have no idea how long I've been trying to figure this out, subbing in item by item to no avail. I feel equal parts stupid and relieved.

edited to add:
So I just logged on to do a quick map run to nab some gloves, seeing as I had spent well over an hour switching gear out and trying to get the damned recipe to work. About a minute and a half into the run, I get a chaos orb drop. It's not quite a Murphy's Law thing, but it's close.
